Harrisburg Branch AAUW The Beverly J. Smith Memorial Scholarship The forms needed to apply for this scholarship are included in one PDF file. (Format requires Adobe Reader; you can download Reader free.) The seven-page document includes the:  Scholarship announcement Application form Required essay question Consent to release educational records Financial aid form Reference form Use the consent form to request verification of enrollment, an official transcript, and the completed financial aid form from your academic institution.  Submit forms for your two personal references (at least one of which must be academic) along with the application form and the essay. One reference form is provided, which may be duplicated.  Applicants for this scholarship are urged to consider applying for the Martha M. Dohner Memorial Scholarship as well, if they meet the requirements. The Smith scholarship winners will be selected first, so there is nothing to be lost in applying for both scholarships.
